The Member Eligibility Confirmation contains important health plan information, including your:

• Name and Member ID Number
• Address, Date of Birth
• Group Number
• Primary Care Physician (PCP), if applicable
• Copay Information
• Designated Radiologist, if applicable
• Vytra Health Plan Type (i.e. HMO, PPO, POS, etc.)
• Vytra Customer Service and Provider Service phone numbers.
